Standard Facility Charges

Eye Surgicenter of New Jersey makes its standard facility charges available to the public. Standard charges are the facility’s established charges for services and are not necessarily the amount a patient or health plan will pay.

A patient’s actual financial responsibility may differ based on insurance coverage, contracted rates, deductibles, copayments, coinsurance, authorization, medical necessity, services actually furnished, and other benefit provisions. Professional Services Are Billed Separately

Physician and other professional services provided in connection with care at the Surgery Center are not included in the facility’s charges and may be billed separately. The physicians and other professionals who provide services at the Surgery Center may not participate in the same health benefit plans as the facility. Patients should contact the applicable professional and their health plan to determine network participation and anticipated financial responsibility.

Anesthesia services are professional services and may be billed separately from the Surgery Center’s facility charge. Patients should verify Centurion Anesthesia’s participation with their specific health plan and product.

Uninsured and Self-Pay Patients

Uninsured and self-pay patients may request a Good Faith Estimate of expected charges.
