Important Information For Patients
When you have an appointment at any one of our offices you are authorizing the release of medical information necessary to process insurance claims to insurance companies or their agencies (including Medicare), for purposes of filing and payment of medical claims. You are then authorizing the office to receive payment of medical benefits. You, as the patient, should understand that you are financially responsible for charges not covered, co-insurance charges, deductibles and non-covered services. Proof of insurance does not guarantee coverage.
Our offices accept cash, check, and most charge cards.
